How do I file a workplace complaint?

By Verity Workplace. Information for US workers. Sources checked .

Quick answer: Start with what happened, when and where it happened, who was involved, and what you want addressed. Keep the original details and any supporting records. Choose whether to report through your employer, a union, or an outside agency. A private Verity record is a place to prepare; creating it does not file a government complaint or notify your employer.

How do you start?

You do not need to know the legal name for the problem. Write one event first: “On Tuesday, my supervisor removed two hours from my timesheet after I had worked them.” Add the date as accurately as you can. If it happened repeatedly, make a separate entry for each event instead of combining everything into one accusation.

  • Write the facts in plain words.
  • Add the dates, people and supporting records you know.
  • Choose whether to keep it private or share a version you reviewed.

What should you document?

A useful complaint lets another person follow the story. Mark estimates as estimates and distinguish what you saw from what someone told you.

  • The incident date, location and most recent occurrence.
  • What was said or done, including quotes you remember.
  • People involved and witnesses, if any.
  • Messages, pay records, schedules or photos you are allowed to keep.
  • Any earlier report, the recipient and the response.
  • What you want reviewed, corrected or explained.

Employer reporting or government filing?

An internal complaint asks the organization to respond. A government complaint goes to an agency through its own process. Agencies have different coverage, information requirements and time limits. OSHA provides workplace safety reporting information, the EEOC provides discrimination reporting information, and the Department of Labor provides wage and leave resources.

You can read official outside options without sending anything through Verity. Do not assume an employer review pauses an outside deadline. Check the relevant agency directly while deciding what to do.

What mistakes should you avoid?

A clear account is more useful than a legal label you are unsure about.

  • Presenting an estimate as an exact date or amount.
  • Changing original messages or screenshots.
  • Sending a version before checking which details identify you.
  • Assuming saving with Verity files with an agency or pauses a deadline.

Where Verity fits

Write your complaint in your own words, add the key details, then choose what happens next. Your original record stays private. Nothing is sent to your employer until you review the employer version and choose to send it. Employees can create, keep and download their record for free.

Verity is an independent record service, not a government agency, law firm or emergency service. It does not file for you, choose an agency, give legal advice or guarantee anonymity. You choose whether to share with your employer and whether to use your record elsewhere.
