How do I report unsafe working conditions?

By Verity Workplace. Information for US workers. Sources checked .

Quick answer: Describe the hazard, where it is, when you observed it, and who may be exposed. OSHA provides a safety and health complaint process and a separate whistleblower process for certain retaliation concerns. Check OSHA or the applicable state program for instructions. If there is immediate danger, use emergency or urgent safety channels rather than waiting for a Verity employer response.

How do you start?

Describe the condition instead of only saying the workplace is unsafe. For example: “The exit beside the loading area was blocked by stacked boxes during the afternoon shift.” Say how often you saw it and whether it is still happening.

Do not enter a dangerous area to gather evidence. A note about what you already observed can be useful without a photo. Verity is a record tool, not an emergency service.

What should you document?

Details about place, timing and exposure help distinguish a continuing hazard from a past event.

  • The worksite address and the exact area involved.
  • The equipment, material or condition creating the concern.
  • The date observed and whether the condition continues.
  • Which tasks or workers may be affected.
  • Photos or messages obtained safely and that you are allowed to keep.
  • Earlier safety reports and what changed afterward.

Safety complaint or retaliation complaint?

OSHA distinguishes reporting a workplace safety hazard from reporting retaliation for protected activity. If both happened, keep the hazard record and the later treatment changes separately. The reporting processes and deadlines can differ. Check the official instructions directly; Verity does not select or submit either filing.

What mistakes should you avoid?

Your record should not delay an urgent safety response.

  • Waiting for an employer unlock when there is immediate danger.
  • Taking risks to obtain a photo or document.
  • Describing only a conclusion without the location or condition.
  • Assuming safety and retaliation reports use the same process or deadline.

Where Verity fits

Write your complaint in your own words, add the key details, then choose what happens next. Your original record stays private. Nothing is sent to your employer until you review the employer version and choose to send it. Employees can create, keep and download their record for free.

Verity is an independent record service, not a government agency, law firm or emergency service. It does not file for you, choose an agency, give legal advice or guarantee anonymity. You choose whether to share with your employer and whether to use your record elsewhere.
